The following conformance statement in the Assessment Performed (V4) template has incorrect template id extension value "2020-08-01". The correct extension is "2021-08-01". 

b. SHALL contain exactly one [1..1] @extension="2020-08-01" (CONF:4509-28701).

The template id extension value in Figure 23: Assessment Performed (V4) Example is correct. The template id extension value directly under the 3.6 Assessment (V4) heading is also correct: "[observation: identifier urn:hl7ii:2.16.840.1.113883.10.20.24.3.144:2021-08-01 (open)]".
