The description in the template is wrong and will be changed from: "This template MAY be included zero or one times [0..1] in an Encounter Activity (V3) template." "This template MAY be included zero or many times [0..*] in an Encounter Activity (V3) template." Will also update description to be clear that only one reason should be included per template, from: "This template represents the problem that is the reason for the manual initiation of the eICR CDA document. " to: "This template represents a problem that is a reason for the manual initiation of the eICR CDA document. If there are multiple reasons then each reason should be contained in a separate Initial Case Report Manual Trigger Observation template."
